A fundamental role of disability support workers is to work in partnership with individuals with disability to plan for, and then implement, individualized plans. A significant aspect of the disability support worker’s role is to ensure the person’s support requirements are met in a way that is responsive to their needs and preferences. The emphasis of an individualized plan is on goals and activities that foster the individual’s participation and independence in a meaningful way.

Today, more people who have disabilities choose to remain living in the community and there is a broader range of services and supports available to help them to do this. For those who do require residential care, the environment provided is more homelike and flexible; giving residents more opportunities to stay in control and make choices about the ways they are supported. The ways support services are delivered needs to be carefully planned to ensure the individual needs of the client can be met in a way that supports their independence, their preferences and their choices.

This topic will guide you through individual support plans, which you should have some knowledge of from Module 1. It will also cover strengths-based practice and active support, again touching on things you have previously learnt in Module 1, but expanding your knowledge. You will move onto delivery models in the disability sector and learn about some key organizations in the disability industry. Finally you will touch on some core legal and ethical considerations, but this will be addressed in more detail later on in Topic 4.

Strength-based practice is a social work practice theory that emphasizes people's self-determination and strengths. It is a philosophy and a way of viewing clients as resourceful and resilient in the face of adversity. It is client-led, with a focus on future outcomes and strengths that people bring to a problem or crisis. When applied beyond the field of social work, strength-based practice is also referred to as the "strength-based approach".
